Are you a clinician looking to refer a patient for psychiatric medication management?

We are accepting new patients in Massachusetts, New Hampshire, and Florida. Click the Referral button below to start a referral form. Once your referral is received, a member of our administrative team will contact the patient to schedule their initial intake and answer any questions that they may have regarding our psychiatric services. Additionally, if you would like to speak with a clinician directly regarding collaborative care or general inquiries, please call the office. You may also fax any additional documents directly to our secured fax line.
